Two lessons ( not so new, but oft-forgotten in a world of reels, jingoism and instant gratification) emerge from this tragic episode –
a)The Boeing experience ( as detailed in the blog) shows that prioritising growth over stability in strategy and business models drives rapid expansion but undermines long-term resilience. Focusing on market share and revenue often ignores operational robustness, with adverse consequences for the organisation and its stakeholders. Consequences of ” growth over stability” business strategy can lead to tragic operational failures but can also get manifested in exodus of talent, consumer harm and adverse environmental impact. While the executives may try to bury these with media management , shifting goalposts and unrealistic promises, the long term consequence is the erosion of customer trust when ethics are compromised at the alter of shareholder return. Boeing’s cultural and credibility damage exemplifies these risks, requiring transparent reforms, enhanced risk management, and safety-focused innovation. At the end, balanced growth, integrating ethical practices and diversified revenue, ensures sustainable, enduring success.
b) The neglect of risk management can harm organisations throughout their lifespan. The tenures of agents (executives, CEO) are significantly shorter than an organisation’s lifespan, so their incentive structures often prioritise short-term gains. Risk managers, as prudent advisers, can make a significant difference by tempering the aggressive risk appetite that often dominates the “tone at the top,” provided they are given a proper seat in the institutional decision-making process. Similarly, truly independent board directors with relevant domain expertise can contribute to this effort. Cultivating a culture of risk awareness, which Boeing clearly failed to instil, can enhance the health and longevity of organisations, not only in aviation but across industries.

Boeing, a preeminent entity in aerospace innovation on a global scale, has consistently represented the pinnacle of American engineering proficiency. With a legacy that spans more than a century, the organization has been instrumental in the evolution of contemporary aviation. However, in recent years, a multitude of catastrophic events—including the 737 MAX disasters, the 2025 Ahmedabad plane crash, yesterday’s flap malfunction in a Boeing 787 Dreamliner necessitating a British Airways flight’s return to Heathrow, and troubling whistleblower accusations in 2024—have triggered significant concerns and undermined public trust and investor confidence. It is imperative that these warning signs pertaining to Boeing’s operational risk management frameworks and organizational ethos are not trivialized through standard public relations tactics. A comprehensive examination of Boeing’s operational risk evaluation throughout the product life cycle is essential, encompassing supervision of suppliers within the supply chain. Furthermore, occurrences that cast doubt on sound governance and risk culture are equally disconcerting. The Federal Aviation Administration (FAA) has repeatedly scrutinized the operational robustness of Boeing aircraft; however, it is evident that more substantial measures must be implemented.
This paper investigates Boeing’s historical trajectory, recent crises, root causes of its risk failures, current risk management practices, and the imperative for systemic reform.
Past
Established in 1916 by William E. Boeing in Seattle, Washington, the Boeing Company initially focused on the production of seaplanes for military and postal applications. The organization’s entry into commercial prosperity commenced with the launch of the Boeing 707 in the 1950s, followed by an array of renowned aircraft, including the 727, 737, 747, and subsequently the flagship 787 Dreamliner. Throughout the latter part of the 20th century, Boeing solidified its position as a preeminent entity in the realm of commercial aviation, engaging in fierce competition with Europe’s Airbus. Strategic consolidations, notably the acquisition of McDonnell Douglas in 1997, facilitated Boeing’s enhancement of its defence and aerospace proficiency. Nonetheless, in recent years, operational misjudgements and governance challenges have adversely affected its reputation on the global stage. This prompts an inquiry. What transpired? Has the leadership culture deteriorated? Has the organization expanded beyond the capacity for effective operational management? Did Boeing elevate its risk tolerance to bolster its financial performance, especially in the aftermath of the global financial crisis?
Performance Crisis and Operational Failures
Boeing’s crisis began to unfold with the 737 MAX program. Designed to compete with Airbus’ A320neo, the aircraft included the Maneuvering Characteristics Augmentation System (MCAS), a software tool intended to improve handling. However, MCAS logic errors are widely believed to have caused two devastating crashes—Lion Air Flight 610 in 2018 and Ethiopian Airlines Flight 302 in 2019—killing 346 people. Investigations revealed that MCAS relied on a single angle-of-attack sensor, and Boeing failed to adequately train pilots or disclose risks to regulators. The resulting fallout led to a global grounding of the 737 MAX fleet and intense scrutiny of Boeing’s engineering, risk assessment, and compliance practices.
Tragedy struck again on June 12, 2025, when Air India Flight AI171, a Boeing 787-8 Dreamliner, crashed shortly after takeoff in Ahmedabad, India. Preliminary data indicates loss of power and thrust less than a minute after takeoff and a maximum altitude of only 625 feet. Although the cause is still under investigation, the incident raises grave concerns about the Dreamliner’s quality assurance and operational risk management processes, particularly in light of earlier multiple red flags.
Whistleblowers
Boeing encountered escalating challenges in 2024 following numerous whistleblower revelations. A seasoned engineer from Boeing submitted a formal grievance to the Federal Aviation Administration (FAA), asserting the existence of structural deficiencies in the fuselage joints of the 787 Dreamliner. The whistleblower indicated that production efficiencies and managerial pressures jeopardized the aircraft’s long-term safety. In a parallel situation, a contractor associated with Spirit AeroSystems alleged wrongful termination for disclosing manufacturing flaws in the forward pressure bulkhead of the 787. This individual expressed significant concerns regarding the safety assurances provided by Boeing and its supply chain partners.
Perhaps most tragically, John Barnett, a former quality manager and long-serving Boeing employee, tragically took his own life in March 2024, days before he was scheduled to provide testimony in a whistleblower retaliation lawsuit. Barnett had consistently raised alarms regarding falsified quality inspections and untraceable non-conforming components at Boeing’s North Charleston facility. His passing, under questionable circumstances, has reignited scrutiny regarding Boeing’s internal culture and the treatment of those who dissent.
Root Cause Analysis
At the heart of Boeing’s crises lie multiple systemic failures. Key among them is the inadequate identification and assessment of operational risks. The MCAS issue was not subjected to rigorous stress-testing, and its critical reliance on a single sensor was overlooked. Likewise, whistleblower accounts raise serious doubts about lapses in quality control during the Dreamliner’s production. Organizational culture also appears to have played a pivotal role. Many analysts and researchers observed that Boeing’s focus on shareholder value, cost efficiency, and market competition led to an environment where operational risk considerations were secondary to schedule pressures.
Opacity as a Red Flag
Communication breakdowns worsened the situation. Boeing’s opacity with pilots, regulators, and airline customers—particularly regarding the MCAS—resulted in adverse consequences. The quality of regulatory oversight also raises concerns , with the FAA depending y on Boeing’s internal safety assessments rather than independent evaluations. Leadership failures also contributed, with decision-makers prioritizing financial metrics over engineering integrity, allowing safety-critical warnings to be dismissed or ignored.
Boeing’s Risk Management Practices
According to its 2023 Annual Report, Boeing utilizes an Enterprise Risk Management (ERM) framework aligned with COSO standards. This model includes risk identification, impact analysis, and mitigation strategies overseen by the Board’s Audit Committee. Boeing also claims to integrate a Safety Management System (SMS) to detect safety issues using data analytics, supplier audits, and process checks.
However, critics argue that Boeing’s risk management remains reactive. Academic research and industry analyses show that Boeing failed to recognize or act on risks proactively, especially with the 737 MAX’s MCAS system. The Columbia University School of Professional Studies noted in 2024 that Boeing’s risk prioritization favored delivery timelines over safety margins. The company’s decentralized risk structure diluted accountability, as documented in The Verge and IEEE Spectrum, leading to missed opportunities to avert disasters.
Supply chain vulnerabilities further complicate Boeing’s risk landscape. Spirit AeroSystems and other subcontractors have been implicated in producing substandard components, as revealed by whistleblower accounts. While Boeing emphasizes collaboration with suppliers, these relationships often lack rigorous independent oversight.
Ineffective Risk Governance
Despite Boeing’s long-standing reputation as a leader in the aerospace industry, the company’s development and rollout of the 737 MAX exposed deep-rooted deficiencies in its approach to risk. Bensoussan ( 2022) highlights that Boeing’s organizational structure and decision-making culture prioritized short-term financial gains over long-term safety and sustainability. Critically, he notes that risk management was virtually nonexistent in Boeing’s organizational chart, a startling omission for a company involved in such a complex and safety-critical industry. This lack of formal, integrated risk management contributed directly to design flaws, regulatory oversights, communication failures, and ultimately, the loss of lives in multiple crashes. When one looks at executive biographies in Boeing.com, there is no Chief Risk officer there. A junior VP is assigned the role of a CRO, with apparently limited impact on organization decision making. The most glaring sign of Boeing’s negligence was its failure to establish a dedicated risk management function or culture. In high-reliability organizations like those in aerospace or nuclear industries, risk management is not merely a support function but a core pillar of operations. However, Boeing’s internal focus during the development of the 737 MAX was centered almost entirely on cost reduction, speed to market, and maintaining market dominance against Airbus (Bensoussan ,2022). Observers have pointed out that Boeing’s decision to develop the MAX as an update to the existing 737 platform, rather than a new aircraft, was driven by the desire to avoid costly pilot retraining and lengthy certification processes. This approach introduced critical design challenges, such as integrating larger, more fuel-efficient engines, which altered the aircraft’s aerodynamics in ways that increased the risk of stall under certain flight conditions.
Redundancy and Fault Tolerance
To mitigate aerodynamic instability, Boeing introduced the Maneuvering Characteristics Augmentation System (MCAS), a software-based solution that was supposed to correct the aircraft’s pitch automatically during steep climbs. However, this system was poorly designed and inadequately tested. Astonishingly, the MCAS software relied on data from only a single angle-of-attack (AoA) sensor, despite the aircraft being equipped with two. This violates basic principles of redundancy and fault tolerance that are standard in life-critical systems. Furthermore, the MCAS could override pilot input repeatedly, even if it was triggered by faulty sensor data. This lack of transparency and control rendered pilots effectively powerless in the face of unexpected MCAS activation, a scenario that played out fatally in both the Lion Air and Ethiopian Airlines crashes.
Inadequate Response to Early Warning Signals
The absence of robust risk assessment procedures is evident in how Boeing handled early warning signs. During simulator tests, a Boeing pilot experienced significant difficulty managing MCAS and described the situation as “catastrophic.” Despite this, Boeing did not adequately revise the system or inform pilots and airlines of the risk. The MCAS was omitted from the pilot training manuals, and Boeing promoted the idea that transitioning from the 737 NG to the MAX would require minimal additional training. This was a deliberate move to reinforce the narrative that the 737 MAX was not substantially different from its predecessor, thereby securing quicker sales and avoiding regulatory hurdles. However, this also meant that pilots were unprepared for MCAS-related malfunctions, as tragically demonstrated in both fatal accidents.
Comparing the Competitors from a Risk Lens : Boeing vs Airbus
Boeing and Airbus, as leading aerospace manufacturers, employ distinct risk management strategies to address safety, supply chain, financial, and operational challenges. Boeing’s Safety Management System (SMS) integrates data analytics and predictive modeling, supported by Design Build Safety reviews to ensure defect-free production. However, its safety record has been marred by the 737 MAX crises (2018-2019), which exposed a risk-taking culture and inadequate training, costing $22 billion in market value. Post-crisis, Boeing introduced a safety board and chief safety officer, but persistent issues highlight ongoing quality control and supplier oversight challenges. In contrast, Airbus maintains a proactive SMS, leveraging its strong safety record with the A320 family and transparent accident analysis. Its diversified portfolio across commercial, defense, and helicopter divisions reduces safety-related financial risks, contributing to a 28% profit increase and a market value of $103.26 billion in 2023.
Third Party Risks are Critical
In supply chain risk management, Boeing’s heavy reliance on outsourcing, particularly to suppliers like Spirit Aerosystems, has led to cost efficiencies but also quality issues, as seen in faulty 737 MAX components. Its Risk-Sharing Partnership program externalizes risks, yet single-supplier concentration increases vulnerability to disruptions. Airbus, while also using Risk-Sharing Partnerships, benefits from a geographically diversified supply chain across Europe, North America, and Asia-Pacific, mitigating concentration risks. Despite a backlog of over 8,600 orders, Airbus adopts cautious production ramp-ups to maintain quality, ensuring compatibility with weaker suppliers.
Silos
Operationally, Boeing’s risk-taking culture has led to strategic missteps, with siloed data collection hindering comprehensive risk analysis. Its point-to-point model aligns with certain market demands but lacks the flexibility of Airbus’s hub-and-spoke approach. Airbus’s conservative strategy prioritizes industry stability over aggressive market share gains, while its ESG focus on zero-emission aircraft reduces regulatory risks. The table below summarizes these differences, highlighting Boeing’s reactive approach and Airbus’s proactive, diversified risk management.
Regulation under Scanner
Moreover, the failure was not limited to Boeing’s internal processes but extended to its relationship with the Federal Aviation Administration (FAA). Anaysts in civil aviation industry describes a situation in which FAA managers, facing resource constraints, delegated significant portions of the aircraft’s safety certification back to Boeing engineers. This created a profound conflict of interest. Boeing personnel, acting as authorised representatives of the FAA, were placed in the position of evaluating their own company’s designs. When concerns about MCAS and sensor vulnerabilities were raised internally, they were not communicated effectively to the FAA. Even when the MCAS system was modified to be more aggressive and to activate at lower speeds, these changes were not disclosed. This regulatory breakdown allowed the flawed system to pass through certification unchecked.
After the first crash involving Lion Air, Boeing’s response further demonstrated its disregard for comprehensive risk management. Instead of grounding the aircraft and conducting a transparent investigation, the company initially blamed pilot error and downplayed the role of MCAS. An operations manual bulletin was issued that vaguely referred to AoA sensor issues, with no mention of the MCAS system. This created confusion and a false sense of security among pilots and airline operators. It was not until after the second crash, involving Ethiopian Airlines, that Boeing and global regulators began to take decisive action. By then, the damage had been done—both in terms of human lives and Boeing’s reputation.
From a strategic standpoint, Boeing’s failure to anticipate the competitive threat posed by Airbus’s A320neo also reveals a lack of risk foresight. Rather than preparing proactively, Boeing reacted hastily, rushing the MAX to market. A well-functioning risk management infrastructure would have identified the emerging threat and provided options, including accepting a temporary loss of market share while developing a new, safer aircraft. Instead, the company’s leadership allowed market pressures and shareholder expectations to dictate engineering decisions, resulting in a compromised design that prioritized speed over safety.
The broader lesson from the Boeing case is that effective risk management must be embedded at all levels of an organization—from engineering to strategy to executive leadership. It is not merely about compliance or damage control after the fact; it is about proactively identifying vulnerabilities and making informed decisions that balance opportunity with safety. The Chief Risk Officer (CRO) role, absent in Boeing’s structure, could have provided the necessary oversight and independence to challenge unsafe decisions. A CRO empowered to report directly to the CEO and Board of Directors could have flagged the structural and software risks in the 737 MAX, advocated for full pilot retraining, and ensured transparent communication with regulators and customers.
In conclusion, Boeing’s neglect of risk management played a central role in one of the most significant safety failures in modern aviation history. The tragic loss of life, the massive financial toll, and the erosion of trust were not the results of a single flaw or mistake, but of a systemic failure to embed risk management into the company’s DNA. The Boeing 737 MAX crisis serves as a cautionary tale for all organizations: risk management is not optional—it is essential to sustainable success, operational safety, and corporate integrity.
The damage to Boeing’s culture and credibility may be the most enduring legacy of these failures. A once-revered name in aerospace now faces the daunting task of regaining trust—a process that will require transparent communication, demonstrable change, and sustained commitment to safety. To regain its stature, Boeing must implement sweeping reforms. Enhancing its risk management framework, protecting whistleblowers and addressing whistleblower concerns, and instilling a safety-first culture are paramount. Technological investments in predictive safety analytics, quality assurance automation, and advanced training for engineers and pilots will be essential. Equally important is a genuine engagement with stakeholders—regulators, airlines, and the public—through consistent, honest communication and a commitment to ethical leadership.
